One of the sentences I most often hear from parents in my clinical practice is undoubtedly this: “It’s going to fall out anyway, do we really need to get this tooth filled?” As a pediatric dentist (pedodontist), my answer to this question has always been a clear "Yes". Pediatric dentistry (pedodontics) is a specialty that protects oral and dental health from infancy through young adulthood, treats diseases, and, most importantly, aims to help children develop a healthy brushing routine that will last a lifetime.
The Misconception of "It’s Going to Fall Out Anyway": Why Are Baby Teeth Important?
Children have a total of 20 baby teeth, and they usually start erupting from around 6 months of age and are complete by 2.5 years. Contrary to the common misconception in society, the fact that these teeth will fall out at a certain age does not mean they are unimportant. Baby teeth directly affect a child’s proper and balanced nutrition, correct pronunciation while learning to speak, and jaw-facial development. More importantly, they "guide" the permanent teeth that will come in beneath them. A baby tooth that decays early and has to be extracted can cause the adjacent teeth to shift into that space, blocking the path of the permanent tooth and paving the way for problems that may later require serious orthodontic treatment.
When Should the First Dental Examination Be Done?
We recommend the first dental visit when the first tooth erupts in babies, usually between 6 months and 1 year of age. The main purpose of this very early visit is not treatment, but to educate the family about nutrition and proper oral care, and to help the baby meet the dentist’s chair "without fear". Routine checkups performed before toothache, swelling, or trauma occur are the most effective way to prevent "dental phobia" that may develop later and make treatment much more difficult.
Preventive Dentistry Saves Lives
Our biggest priority in pedodontics is not to carve out the tooth and fill it after decay has formed, but to ensure that decay never starts in the first place. At this point, we have two powerful preventive tools: Fluoride applications and fissure sealants.
Fluoride, which we apply to the tooth surface with the help of special varnishes, strengthens the enamel layer and creates a shield against cavities. Fissure sealants (dental sealants), on the other hand, completely prevent food from getting trapped and bacteria from accumulating in the deep grooves on the chewing surfaces of the back molars by sealing those areas, especially. These two completely painless and needle-free procedures are the most valuable investment you can make in your child’s dental health. We also educate families about the rapidly progressing "bottle caries" caused by falling asleep with a bottle or nighttime feeding, and we reduce cavity risk by improving eating habits.
How Is Fear of Dental Treatment Overcome in Children?
The clinical environment is stressful even for adults, and for a child it is an even more frightening unknown. We approach every little patient who sits in the chair with the "Tell, Show, Do" technique. We introduce the instruments we will use in a way that is adapted to their world, turning them into a game and earning their trust. Our biggest request from families is that they absolutely do not use words such as needle, extraction, or pain before bringing children to the clinic, and that they do not present the dentist as a means of punishment ("If you misbehave, I’ll take you to the dentist").
Remember: the foundation of healthy, strong, and cavity-free permanent teeth is laid with carefully cared-for healthy baby teeth.
